If you have a csv file, you can easily import the csv and create a table in mysql using Sequel Pro.
As usual, choose File > Import
Choose a csv file
And click on the “New” button to create a new table based on the file structure.

As soon as I click the New button all the fields appear as expected in the window correctly mapped ready for Import, but the Import button becomes greyed out so that I cannot proceed. What’s going on?

Maybe there are some fields not correctly matched. Or some field names consist of empty space.
Anyway the issue must be due to some errors on the field names
